● Coordinates total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the patient’s condition, promoting sound preventive practices, coordinating services and teaching.
● Develops and implements the nursing care plan.
● Coordinates patient services.
● Use the nursing procedure to assess, plan, implement and evaluate patient care.
● Performs focused interview to identify specific patient needs.
● Assess signs and symptoms indicating changes in the patient’s condition.
● Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
● Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
● Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
● Uses clinical judgment in evaluation activities to meet patient care needs and establish priorities.
● Other Registered Nurse (RN) duties as assigned.

● Providing one-on-one care in the home
● Aides will be responsible for providing care indicated on the care plan (such as assisting with bathing, hair washing and shaving, dressing assistance, medication reminders, assistance with toileting and other personal care needs) and/or companion care (such as accompanying the client to appointments, running errands such as grocery shopping, activities such as taking walks, playing games and reading, meal preparation, washing dishes, laundry and ironing, light housekeeping and general companionship)
